Fingerprinting and TB Test

For the safety of our children, ALL school volunteers
are required to be fingerprinted and pass a TB test

 

It's as easy as 1-2-3:

1. Contact your physician for a TB test. Allow for the 48-hour reading requirement (you will have to return to the lab for the test).

 

2. Contact the MDUSD at 682-8000, ext. 4153 to schedule your fingerprint appointment. Fingerprinting must be conducted by the district, at the  district offices.

  • Proof of negative TB cannot be older than 60 days.
  • Previous fingerprint records from other agencies are not acceptable; there is no cross-agency reciprocity when it comes to checking and monitoring fingerprinted individuals.
  • Fingerprinting must be done at the district offices; their machines are not mobile and so cannot be brought to Prep Night, etc.

 

3. Bring both the Fingerprint Form and your TB test results to your appointment. (There is also a charge for fingerprinting, ~$32 CASH ONLY). For the form line stating “fieldtrip/classroom”, fill in Mt. Diablo Elementary School and your student's name.

This clearance must be done only by our district, no exceptions!

Please schedule your TB and fingerprinting appointments immediately, as it sometimes takes longer than a couple of weeks to complete this process due to the overwhelming number of supportive parents wishing to be a part of their student's educational experience.

You only need to be fingerprinted through the District ONCE, but every 4 years you must submit a new TB test. It is the parent's responsibility to ensure the TB is current - If your TB has expired, please update and turn that information into the school.  If you are unclear of your fingerprinting or TB status, please check with the office.
